Driver Authorization Form

Bethel Vehicles are rolling advertisements for the University. Safe and respectful driving habits reflect well on the University. Bethel Drivers are expected to adhere to the following conditions.

Conditions

  • Employees must renew this form every 4 calendar years. Students must renew the form every two school years.
  • Any driver with the following violations in the past 3 years will not be allowed to operate a Bethel vehicle
    • 3 or more moving violations
    • 2 or more at-fault accidents
    • Careless driving
    • Passing a stopped school bus
    • Texting while driving
  • Any driver with the following violations in the past 3 years will not be allowed to operate a Bethel vehicle
    • Reckless driving
    • Speed in excess of 25 MPH over the posted limit
    • Driving while intoxicated or under the influence
    • Open container (alcohol)
    • Drag racing
    • Leaving the scene of an accident
    • License suspension
    • Vehicular homicide
    • Criminal-type conviction
  • Cell phone use is STRICKTLY PROHIBITED while driving any Bethel vehicle for phone calls or text messages.
  • Drivers of Commercial Motor Vehicles (CMVs) must have a complete driver qualification file with the Transportation Office and comply with all applicable DOT regulations.
  • Any driver found to be grossly negligent while operating a Bethel vehicle will be liable for Bethel's deductivle portion of the insurance claim or repair charges.
  • All drivers of Bethel vehicles are responsible for ensuring that the capacity limits are strictly adhered to at all times and that all passengers wear seatbelts.
