Overview
Minnesota state law requires some students to provide attestation of immunization dates for the following: measles, mumps, rubella, tetanus, and diphtheria. The law makes others exempt from the requirement.
Proof of immunization

Students intending to live on campus must provide proof of immunization to Health Services before moving into their assigned residence their first term as a Bethel student.

Students intending to live off campus must provide attestation of immunization dates when attending face-to-face classes, on-campus residencies, or intensives unless otherwise exempted by Minnesota law.
Students can obtain a medical or non-medical exemption. Students exempt from providing immunization records are as follows:

Any student born before 1956

Any student who graduated from a Minnesota high school in 1997 or later

Students enrolled only in fully online academic program unless required by their program to be immunized
Note: Some programs, courses, field experiences, clinical experiences, or those going on study abroad may have specific immunization requirements students must follow to participate.
To fulfill your proof of immunization requirement:
- Provide your proof of immunization or complete your waiver online through the immunization form in Banner, or
- Complete the Conscientious or Medical Exemption form (pdf) if your personal beliefs or health considerations precluded you from being immunized and you are not eligible for the waiver. Return this form to Health Services. Please note, online submittal is not available as a notary or physician signature is required.

Due dates
The immunization process must be completed no later than 14 days after the start of your first term of registration. After this date, a registration hold will be placed on your student record. Future registration will not be available until the immunization requirement is fulfilled.
Further recommended immunizations
- Meningitis (pdf)—highly recommended for all college students; booster recommended if initial vaccine was given before age 16
- Hepatitis B—three-dose series
- Varicella (Chicken pox)
- Polio
- Influenza—yearly
