All Bethel students must submit immunization records and any medical forms required for certain programs or activities.

Medical Exam for Programs and Activities
A doctor's exam is not required for university admission, except for the following programs and activities:
- Sports medical evaluation (pdf) - For all incoming freshmen and transfer students trying out for and participating in an intercollegiate sport) -
- Nursing applicant medical evaluation (pdf) - For applicants into the nursing program during sophomore year.
Release of Health Information Form
To request your medical records be sent to or from Health Services to another provider, or to grant permission for records to be shared with a parent/guardian, please complete and submit a Release of Health Information Form (pdf).
Parental Consent Form
All students under 18 years of age need parental consent for Health Services to provide medical care.
Health Insurance Requirement
All Bethel undergrad students are required to have health insurance. Learn more about Bethel's health insurance requirement.
